Request a copy of your reported Motor Vehicle Record (MVR)

Please use link below to request a copy of your reported Motor Vehicle Record (MVR) from Safety Holdings, Inc. It will also provide you with the details of any entities that received an MVR report in the two years prior to your request.

IMPORTANT NOTE: YOU WILL NEED TO ATTACH A COPY OF YOUR LICENSE TO PROCEED WITH THIS REQUEST.

Request a Copy of Your Reported MVR

Steps and what to expect:

Step 1. Use link to submit a request for a copy of your recently reported Motor Vehicle Record (MVR) along with a clear picture of your drivers license. The reviewers must be able to read your license clearly in order to process your request.

Step 2. Upon submission of your request form, you will receive a confirmation pop up that your request has been received by the Safety Holdings Compliance Department. Please note that Safety Holdings has 30 days to respond via email and all requests are answered in the order that they are received.

Step 3. Jotform will be sending you an email response. The email will include a password protected copy of your driving record which can be opened by using your drivers license number. You must use exact characters in order to match passwords.

Step 2. Ensure that you initial and sign all fields indicated.

Step 3. Provide a clear copy of your driver’s license. It must be readable for the compliance department to investigate. If you have any supporting documentation, please include that along with your driver’s license.
